MSHSAA updated the Physical Form in April 2023.  Please be sure you are using the new form that is shown to the left.  Steps 1 and 2 (pages 1-3) are to be completed and left with the Physician.  Step 3 (page 5) is to be completed by the physician and RETURNED to the school. 

There are additional forms that need to be filled out that are located in the Parent Portal of PowerSchool under Forms - Athletics.  If you didn't fill these out during Registration, please do so as soon as possible.
